Parent / Guardian Consent

  1. I confirm that I am the parent or legal guardian of the participant named in this registration and that I give permission for them to participate in activities organised by Crawley Community Table Tennis Club (CCTTC).
  2. I confirm that the information provided during registration is accurate and that I will inform CCTTC of any relevant changes or information that may affect the participant's participation.
  3. I understand that CCTTC is a community table tennis club and that sessions may involve coaching, practice, recreational play and competitive activities appropriate to the participant's age and ability.
  4. I understand that CCTTC will take reasonable steps to provide a safe and appropriate environment and that participants are expected to follow the club's rules, safeguarding requirements and codes of conduct.
  5. I authorise CCTTC to contact me regarding the participant's membership, bookings, sessions, coaching, competitions, safeguarding matters and other club-related matters.
  6. I understand that CCTTC may need to contact me in an emergency.
  7. By selecting "I agree", I confirm that I am authorised to provide this consent on behalf of the participant.

Photography and Video Consent

Crawley Community Table Tennis Club (CCTTC) may occasionally take photographs or video recordings during club sessions, coaching, competitions, events and other club activities.

These images or recordings may be used to promote and celebrate the club and table tennis, including on the CCTTC website, social media channels, newsletters, promotional materials and other official club communications.

For participants under 18, we will only use identifiable photographs or video recordings for these purposes where appropriate parent/guardian consent has been provided.

I understand that consent can be withdrawn at any time by contacting CCTTC. Withdrawal of consent will apply to future use of images and recordings, although it may not be possible to remove material that has already been published or distributed.

CCTTC will take reasonable steps to ensure that photographs and video are appropriate, respectful and used only for legitimate club purposes. Personal information such as home address, telephone number or other private information will not be published alongside an image.

For participants under 18: I confirm that I am the participant's parent or legal guardian and that I am authorised to provide this consent.
